Amount of Grant:
|
|
One award in the amount of $1,000 will be given in support of tuition, travel and expenses for participation at a continuing education event. Once approved, the award recipient will forward their event application to CCMC for processing. The remainder of the award will be distributed to the recipient in the form of a cash award. |

Open to all current CCM’s, excluding current and past CCMC Commissioners |

The applicant must be currently working in the field of Case Management

Two letters of recommendation from a colleague or supervisor highlighting the applicant’s commitment to continuing education and mentoring is to accompany the application.

The applicant must agree to the use of his/her name and a promotional quote by CCMC to promote this program in future years.
|
| • |
Applicants must be willing to work with CCMC’s public relations writer to author an article for potential publication on CCM website, newsletter or external publication. The article will highlight the individuals commitment to continuing education, application of learning to practice, the impact of mentoring on professional development, or other related topics approved by CCMC.

One complete original and six (6) blinded copies of the grant application. Blinded copies include no references to the individual name, employer, or work setting.

| 2. |
Two letters of recommendation.
|
| 3. |
An essay describing the applicant’s vision for professional case management and how it would be supported by participation in the proposed event.

Deadline

| 1. |
Completed applications must be postmarked by July 1, 2008. It is strongly recommended that applications be mailed via registered/certified mail or through an overnight carrier.

No incomplete applications will be considered.
|
| 3. |
No faxed copies will be accepted.
|
| 4. |
Applications will be reviewed by CCMC and decisions by the committee will be final.
|

Each application will be assigned a review number by the national office. The applications will then be distributed to the review panel for blind review. The application will be scored blindly according to predefined criteria.

| Notification |
Candidates will be notified of the review panel’s decision in October of each year in celebration of Case Manager Week. The award recipient may use the award for the approved event anytime within the 12 months following receipt of the award.
